Matrix Medical Network seeks Nurse Practitioner providers to conduct comprehensive in-home assessments, reviewing health history, medications, and risks to improve outcomes.

Providers work in members' residences or SNFs, with flexible scheduling and compensation tied to assessments completed. Current NP licensure and prescriptive authority are required, along with relevant board certification.

Competenze

• Must hold NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Gerontology or Family NP.
• Current BLS/ACLS/CPR certification.
• Valid state driver’s license.
• Experience: 1 year NP preferred; new grads welcome.

Mansioni

• Conduct adult/geriatric assessments with medical history, diagnoses, health education, referrals, and documentation per Matrix guidelines.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family, and ancillary staff as appropriate.
• Provide services at the member’s residence which can be in homes or Skilled Nursing Facilities.
