Job summary

Matrix Medical Network is seeking a Nurse Practitioner to perform Comprehensive Health Assessments in members' homes or Skilled Nursing Facilities. Visits can last up to one hour and focus on health history, medications, social factors, and overall well-being.

You will administer tests, coordinate with physicians and case managers, and provide patient education and follow-up. The role offers flexible scheduling and potential for full-time opportunities.

Qualifications

• Master’s Degree or commensurate experience and satisfactory NP licensure.
• Current NP licensure in state of practice including prescription authority.
• AANP, AACN, or ANCC board certification in Adult/Adult Gerontology Family or Emergency care.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads welcome.
• Valid state driver’s license for state of residence.
• Automobile insurance coverage as required for state of residence.
• Strong computer skills and familiarity with health/medical record software.
• Excellent verbal and written communication skills.

Responsibilities

• Review and observe a member’s health, medical history, medication adherence, and social environment during a visit up to one hour.
• Administer and order point of care and lab testing.
• Collaborate with PCPs, case managers, social workers, and family members as needed.
• Provide education and follow-up in coordination with PCP.
• Deliver services at the member’s home or Skilled Nursing Facility.
