Job summary
Matrix Medical Network seeks qualified Nurse Practitioners to perform Comprehensive Health Assessments in members' homes or Skilled Nursing Facilities. Each visit lasts up to one hour and captures health history, medication adherence, and social factors to reveal true health status.
You will collaborate with physicians, case managers, and staff to educate patients, order tests, and follow up to close care gaps.
Qualifications
- Master’s Degree in Nursing or commensurate NP experience.
- Current NP licensure in state of practice with prescriptive authority.
- Board certification in Adult/Family/AGNP/ACNP/EM or related specialty.
- Current BLS/ACLS or CPR certification.
- 1 year NP experience preferred; new grads welcome.
- Valid state driver’s license and auto insurance.
- Strong computer skills and familiarity with health record software.
- Excellent verbal and written communication skills.
Responsibilities
- Conduct adult/geriatric assessments including history, diagnosis and treatment planning.
- Administer and order point-of-care and lab-based testing as needed.
- Collaborate with physicians, case managers, social workers, and caregivers.
- Coordinate with PCP on patient education and follow-up.
