Job summary

Matrix Medical Network hires Nurse Practitioners to perform Comprehensive Health Assessments in members’ homes, including geriatric evaluations and health education. Visits can last up to one hour and may occur at homes or Skilled Nursing Facilities, with coordination from physicians and care teams.

Providers review medical history, medications, and risks, order testing as needed, and help close care gaps. Flexible scheduling, competitive pay, mileage reimbursement, and ongoing education support

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice to include prescription authority or ability to obtain prescriptive authority.
• AANP, AACN, or ANCC board certification in Adult, Family or Gerontology specialties.
• Current BLS, ACLS or CPR certification.
• 1 year of NP experience preferred; new grads encouraged to apply.
• Valid state driver's license and automobile insurance for the state of residence.
• Strong computer skills and familiarity with health record software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric health assessments including medical history, diagnosis, treatment, and education.
• Administer and order point-of-care and lab testing as needed.
• Collaborate with PCPs, case managers, social workers, and caregivers.
• Provide patient education and follow-up coordination with the care team.
• Deliver services at the member’s residence, including homes or SNFs.
