Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform Comprehensive Health Assessments in patients’ homes or Skilled Nursing Facilities. Each visit lasts up to one hour and includes review of medical history, medications, adherence, and social factors to identify gaps in care.

You will collaborate with physicians and care teams, order tests as needed, and provide clear documentation to improve health outcomes. Flexible scheduling and opportunities for growth are offered.

Qualifications

• Master’s Degree or commensurate NP experience with licensure.
• Current NP licensure in state with prescriptive authority.
• Board certification in Adult, Gerontology, Family, Emergency, or Acute Care NP specialties.
• BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged.
• Valid state driver’s license and auto insurance.
• Strong computer skills and familiarity with health records software.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, treatment, and referrals.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, and caregivers.
• Coordinate with PCP for education and follow-up.
