Job summary

Matrix Medical Network offers in-home health assessments conducted by Nurse Practitioners. Providers review health history, medications, and social factors to identify gaps and coordinate follow-ups with PCPs and care teams.

Assessments occur at the member's residence, including homes and Skilled Nursing Facilities, with flexible scheduling and ongoing education support. Providers may obtain additional licensure as needed, benefiting from competitive pay based on completed assessments and

Qualifications

• Master’s degree or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult or Family NP or related field.
• Current BLS/ACLS or CPR certification.

Responsibilities

• Conduct in-home comprehensive health assessments for older adults and at-risk individuals.
• Review medical history, medications, and social factors to identify care gaps.
• Coordinate with PCPs, case managers, and caregivers to plan follow-ups.
• Administer and order point-of-care and lab-based testing as needed.
• Provide patient education and document findings per Matrix guidelines.
• Support licensure expansion and credentialing as needed.
