Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform Comprehensive Health Assessments in members' homes or Skilled Nursing Facilities. Each visit lasts up to an hour and focuses on health history, medications, social factors, and health risks.

You will administer tests, coordinate with the PCP and care team, and contribute to closing care gaps. The role offers flexible scheduling, competitive pay per assessment, and benefits including 401(k) and licensure support.

Qualifications

• Master's Degree in Nursing or commensurate experience and NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, Family, or Gerontology specialties.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, and treatment.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with PCP, case managers, social workers, and family members.
• Provide services at members' residences (homes or SNFs) and ensure documentation.
