Job summary

Matrix Medical Network offers an opportunity for Advanced Practice Providers to perform comprehensive in-home health assessments. You will review medical history, medications, and social factors, and work to close care gaps during each visit at the member’s residence, including SNF settings when applicable.

We provide flexible scheduling with weekend and evening options, and a path from PRN to FT roles. Compensation is tied to per-assessment rates and monthly visit averages, with mileage

Qualifications

• Master’s Degree in NP or commensurate experience with NP licensure.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult, AGPCNP, AGACNP, Family, Emergency, or Acute Care.
• Current BLS, ACLS or CPR certification.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric health assessments with medical history, diagnoses and treatments.
• Administer and order point-of-care and lab tests as needed.
• Collaborate with physicians, case managers, social workers and family members.
• Coordinate with PCP for education and follow-up.
