Job summary

Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive health assessments in patients’ homes, with focus on geriatric and adult populations. You will review medical history, medications, and risk factors, and coordinate with PCPs and care teams to close care gaps.

Responsibilities include performing assessments, ordering tests, coordinating with providers, and delivering care at home or in SNFs.

Qualifications

• Master’s Degree in NP or equivalent required.
• Current NP licensure in practicing state.
• Board certification in Adult/Family/Gerontology nursing.
• BLS/ACLS/CPR certification current.
• 1 year NP experience preferred.
• Valid state driver’s license.
• Strong computer skills and familiarity with EMR software.
• Excellent verbal and written communication skills.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnosis, and treatment.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, families and caregivers as appropriate.
• Provide patient education and follow-up with PCP on necessary actions.
• Deliver services at the member’s residence (home or SNF).
