Job summary

Matrix Medical Network seeks a skilled nurse practitioner to perform Comprehensive Health Assessments in patients' homes, including review of medical history, medications, and social factors. Visits last up to one hour and may occur Sun–Sat between 8 am and 7 pm, adapting to your schedule.

You will work with physicians, case managers, social workers, and caregivers, arrange education and follow-up, and may assist with licensure expansion.

Qualifications

• Master’s degree or commensurate experience is required.
• NP licensure in state of practice with prescriptive authority.
• AANP, AACN, or ANCC board certification in Adult/Adult Gerontology/Family/ Emergency/Acute Care is required or expected.
• Current BLS, ACLS or CPR certification is required.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Conduct adult/geriatric assessments including medical history, diagnoses and treatment, health education, referrals, coordination and documentation.
• Administer and order point-of-care and lab-based testing as needed.
• Collaborate with physicians, case managers, social workers, family members, caregivers and other clinicians.
• Coordinate with PCP on patient education and follow-up as necessary.
• Provide services at the member’s residence (homes or Skilled Nursing Facilities).
