Job summary

Matrix Medical Network is hiring providers to perform in-home Comprehensive Health Assessments, reviewing medical history and risks at a member's residence or SNF. You will collaborate with physicians, case managers, and caregivers to improve care quality and close care gaps.

Qualifications

• Master’s degree and NP licensure in state of practice.
• Board certification in relevant adult-gerontology or family specialty.
• Current BLS/ACLS/CPR certification and prescriptive authority.
• 1 year NP experience preferred; new grads encouraged to apply.

Responsibilities

• Perform in-home Comprehensive Health Assessments reviewing health history and risks.
• Coordinate referrals, education, and follow-up with PCPs and care teams.
• Administer point-of-care testing and document per Matrix protocols.
• Travel to members' homes or SNFs and maintain accurate records.
