Job summary

Matrix Medical Network is seeking Nurse Practitioners to perform Comprehensive Health Assessments at members' homes or Skilled Nursing Facilities. Each visit lasts up to an hour and evaluates health history, medications, and risks to close care gaps.

You will collaborate with physicians, case managers, and support staff, traveling within your state as needed. Enjoy flexible scheduling, competitive pay, and full-time opportunities.

Qualifications

• Master’s degree in nursing or commensurate NP experience.
• Current NP licensure in state of practice with prescriptive authority.
• Board certification in Adult or Family/Primary Care required.
• Active BLS/ACLS or CPR certification is needed.

Responsibilities

• Conduct adult/geriatric health assessments and document results per Matrix protocol.
• Administer point-of-care and lab tests as needed and order referrals.
• Collaborate with PCP, case managers, social workers, and caregivers for follow-up.
