Job summary

Mercy seeks a Nurse Manager to lead the Medical Management & Transition Team delivering virtual-based transitional care programs. You will oversee operations, clinical quality, and staff performance while driving population health strategies across the care continuum.

The role requires BSN, active state licensure, and 4 years of clinical/management experience, with CPR NRP and a mandatory medication/math competency exam for direct patient care.

Qualifications

• Graduate of an approved school of nursing.
• Current licensure by the State Board of Nursing.
• Four years of clinical experience with management/supervision.
• CPR NRP certification.
• Medication and math competency test for those providing direct patient care.
• BSN or MSN preferred.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual-based transitional care programs.
• Ensure safe, patient-centered care and high-quality outcomes.
• Foster interdisciplinary collaboration and care coordination.
• Oversee staff performance, regulatory compliance, and budgeting.
• Support ED-to-home and population health initiatives.
