Job summary

Mercy seeks a Nurse Manager to lead the Medical Management & Transition Team, guiding virtual-based transitional care programs and population health initiatives. You will oversee operations, quality outcomes, staff performance, and regulatory compliance while driving safe, patient-centered care across the care continuum.

Responsibilities include coordinating interdisciplinary teams, ensuring timely follow-up, and advancing Mercy’s mission through innovative technology and coordinated care

Qualifications

• RN license active in the applicable state of practice.
• BSN or progress toward BSN within 2 years if possible.
• 4 years of clinical specialty experience with management exposure.
• CPR and NRP certifications.
• Medication/math competency test for direct patient care.
• Preferred: BS/MS and 2+ years supervisory/management experience.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual care management programs.
• Oversee daily operations, quality outcomes, staff performance, and regulatory compliance.
• Foster interdisciplinary collaboration and ensure timely follow-up and care coordination.
• Advance population health strategies and support ED-to-Home and transitional care initiatives.
• Ensure safe, patient-centered care delivery and continuous improvement.
