Job summary

Mercy is seeking a Nurse Manager to lead the Medical Management & Transition Team, directing virtual-based transitional care programs across the care continuum. You will oversee operations, quality outcomes, staff performance, and regulatory compliance while driving interdisciplinary collaboration and safe patient-centered care.

Ideal candidates have a BSN (MSN preferred), RN licensure, and at least four years of clinical and supervisory experience.

Qualifications

• Graduate of an approved school of nursing.
• Current licensure by the State Board of Nursing in the applicable state of practice.
• Four (4) years of experience in clinical specialty or related area with supervisory/management exposure.

Responsibilities

• Provide operational, clinical, and strategic leadership for Mercy’s virtual-based transitional care programs.
• Oversee daily operations, quality outcomes, staff performance, and regulatory compliance.
• Foster interdisciplinary collaboration and ensure timely follow-up and care coordination.
• Advance Mercy’s mission through population health strategies and innovative virtual care management.
