Mercy is seeking a Nurse Manager for the Medical Management & Transition Team. This role leads virtual-based transitional care programs and ensures safe, patient-centered care with interdisciplinary collaboration across the care continuum.

Responsibilities include managing daily operations, quality outcomes, staff performance, regulatory compliance, and program expansion to improve patient follow-up and coordination.

Qualificações

• Graduate of an approved school of nursing; BSN or to complete in the next 2 years.
• Current licensure by the State Board of Nursing in the applicable state.
• Four years in a clinical specialty or related area; includes supervision and budget.
• CPR and NRP certifications.
• Medication and math competency test required before medication administration.

Responsabilidades

• Provide operational, clinical, and strategic leadership for virtual-based transitional care programs.
• Oversee daily operations, quality outcomes, staff performance, and regulatory compliance.
• Foster interdisciplinary collaboration and ensure timely follow-up and care coordination.
• Advance population health strategies and Mercy's mission through virtual care.
