Job summary

Mercy is seeking a Nurse Manager for the Medical Management & Transition Team to lead virtual-based transitional care and Mercy Lifestyle and Wellness programs. You will oversee operations, drive quality outcomes, supervise staff, and ensure compliance with regulatory standards while promoting patient-centered care.

In this role, you will coordinate care across the continuum, support performance improvement, and advance Mercy’s mission through population health strategies and innovative remote

Qualifications

• Graduate of an approved nursing program; BSN or enrolled to complete in 2 years.
• Current licensure by the State Board of Nursing in the applicable state.
• Four years of clinical experience with management, supervision, teaching, performance improvement, and budgeting.
• Basic computer skills including data entry, Windows, and MS Office.
• CPR and NRP certifications.
• BS/MS preferred; supervisory/management experience preferred.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual-based transitional care and lifestyle programs.
• Oversee daily operations, quality outcomes, staff performance, and regulatory compliance.
• Ensure safe, patient-centered care delivery and foster interdisciplinary collaboration.
