Job summary

Mercy is seeking a Nurse Manager to lead the Medical Management & Transition Team, overseeing virtual-based care programs focused on transitioning patients across the care continuum.

The role drives operational and clinical excellence, ensures quality outcomes, and manages staff performance and regulatory compliance while promoting collaboration across disciplines and leveraging population health strategies.

Qualifications

• Graduate of an approved school of nursing.
• Current licensure by the State Board of Nursing in the applicable state of practice.
• Four (4) years of experience in clinical specialty or related area with supervision/teaching.
• CPR and NRP certifications.
• Must pass medication and math competency test before medication administration responsibilities.

Responsibilities

• Provide operational, clinical, and strategic leadership for virtual transitional care and wellness programs.
• Ensure safe, effective, and patient-centered care delivery and timely follow-up.
• Foster interdisciplinary collaboration and care coordination across the care continuum.
• Oversee daily operations, quality outcomes, staff performance, and regulatory compliance.
• Support ongoing population health initiatives and program innovation.
