Responsabilités

• Performs review of medication lists to understand and eliminate barriers to therapy and acts as Pharmacy Navigator to improve adherence.
• Facilitates refills, new prescriptions, appointments, and access to medications for patients with financial constraints.
• Collaborates with Healthfirst Pharmacists and partners to coordinate MTM interventions within the broader care management program.
• Monitors member medication adherence by coordinating with community pharmacies and providers.
• Conducts patient health assessments to identify barriers to adherence related to medications and access to care.
• Handles member and provider communications by phone (in person when needed).
• Performs additional duties as necessary.
