Healthfirst seeks a Pharmacy Technician MTM to serve as a member’s Pharmacy Navigator, reviewing medication lists and improving adherence. You will arrange refills, coordinate new prescriptions, and assist with access to medications, while documenting care and coordinating with pharmacists and providers.

You will assess patient medication profiles, communicate plans, and partner with community pharmacies to monitor adherence. Some in-person work may be required to fulfill duties.

Qualificações

• High school diploma or GED required and professional communication skills.
• Experience communicating with patients, providers and pharmacists.
• Nationally certified Pharmacy Technician or licensed in good standing.

Responsabilidades

• Review medication lists to support adherence and reduce barriers.
• Facilitate refills, new prescriptions, appointments, and financial access.
• Communicate MTM plans with internal/external partners.
• Monitor member medication adherence with community pharmacies.
• Conduct patient health assessments and document care delivered.
• Handle member and provider communications by phone (in person may be required).
• Perform additional duties as needed.
