Job summary

Healthfirst is seeking a Pharmacy Technician to serve as a Pharmacy Navigator, reviewing medication lists to remove barriers and improve adherence, while preventing adverse events. You will coordinate refills, new prescriptions, and appointments, and help members access medications despite financial challenges.

You will partner with Healthfirst pharmacists and external partners to communicate treatment plans and MTM interventions within the broader healthcare management services.

Qualifications

• Clear, professional written and verbal communication experience.
• Experience communicating with patients, providers, and pharmacists on medication issues.
• Nationally certified Pharmacy Technician (PTCB/NHA) or licensed/pharmacist registered in good standing.

Responsibilities

• Eliminate barriers to therapy and promote medication adherence as the member's Pharmacy Navigator.
• Facilitate refills, new prescriptions, appointments, and access to medications for those with financial barriers.
• Coordinate MTM interventions within broader health management services with internal and external partners.
• Monitor and communicate member adherence with community pharmacies.
• Conduct patient health assessments to identify barriers to adherence and access to care.
• Handle member and provider communications by phone or in person as needed.
• Perform additional duties as necessary.
