Job summary

Healthfirst seeks a Pharmacy Technician to act as a Pharmacy Navigator, reviewing medication lists to promote adherence and prevent gaps in therapy. Qualifications

• High School Diploma or GED from an accredited institution.
• Experience communicating with patients, providers, and pharmacists on medication and related issues.
• Nationally certified Pharmacy Technician or registered/licensed Pharmacy Technician in good standing.

Responsibilities

• Review medication lists to promote adherence and reduce barriers to therapy.
• Facilitate refills, new prescriptions, appointments, and access to medications for financial barriers.
• Coordinate MTM interventions with Healthfirst pharmacists and external partners.
• Monitor member medication adherence with community pharmacies.
• Perform patient health assessments to identify barriers to adherence.
• Handle communications with members and providers by phone or in person.
• Additional duties as necessary.
