Job summary

Brigade Health is seeking a compassionate Mobile Patient Care Coordinator to connect patients, families, healthcare providers, and community partners for timely, coordinated care. You will coordinate services, handle inquiries, and support care plans across the continuum.

The role emphasizes accurate documentation, telehealth support, and proactive patient engagement in a fast-paced healthcare setting. Prior healthcare exposure is preferred; high school diploma required.

Qualifications

• High school diploma or GED is required.
• Medical Assistant is preferred but not required.
• Strong communication and customer service skills.
• Comfortable using computers, email, and EMR systems.
• Ability to manage multiple priorities with attention to detail.
• Experience obtaining and documenting patient vital signs is strongly preferred.

Responsibilities

• Coordinate patient care and follow-up services.
• Manage incoming calls, emails, and patient inquiries.
• Process provider orders including medications, labs, imaging, referrals, and home health services.
• Communicate care plan updates with patients, families, community staff, and healthcare partners.
• Support medication reconciliation and escalate concerns to providers when needed.
• Assist with post-discharge outreach and ensure follow-up care is scheduled.
• Identify urgent patient concerns and help coordinate timely interventions.
• Maintain accurate documentation and patient records.
• Support telehealth appointments and care coordination activities.
• Obtain and document patient vital signs and communicate abnormal findings to the care team.

Western Geriatrics and Neurology as part of Brigade Health is looking for a compassionate, organized, and detail-oriented Mobile Patient Care Coordinator to join our growing team. In this role, you'll be the connection between patients, families, healthcare providers, and community partners, helping ensure everyone receives timely and coordinated care.

What You'll Do

• Coordinating patient care and follow-up services
• Managing incoming calls, emails, and patient inquiries
• Processing provider orders, including medications, labs, imaging, referrals, and home health services
• Communicating care plan updates with patients, families, community staff, and healthcare partners
• Supporting medication reconciliation and escalating concerns to providers when needed
• Assisting with post-discharge outreach and ensuring appropriate follow-up care is scheduled
• Identifying urgent patient concerns and helping coordinate timely interventions
