Job summary

Western Geriatrics and Neurology, part of Brigade Health, is seeking a Mobile Patient Care Coordinator to connect patients, families, providers and community partners, ensuring timely and coordinated care.

You will be the primary contact for your assigned patient population, coordinating care, managing inquiries, and documenting care plans. Healthcare experience is preferred; a high school diploma is required.

Responsibilities

• Coordinate patient care and follow-up services with care teams.
• Manage incoming calls, emails, and patient inquiries.
• Process provider orders including medications, labs, imaging, referrals, and home health services.
• Communicate care plan updates with patients, families, and care partners.
• Support medication reconciliation and escalate concerns to providers when needed.
• Assist with post-discharge outreach and arrange follow-up care.
• Identify urgent patient concerns and coordinate timely interventions.
• Maintain accurate patient documentation and records.
• Support telehealth appointments and care coordination activities.
• Obtain and document patient vital signs and report abnormal findings to the team.

What You'll Do

As a Patient Care Coordinator, you'll serve as the primary point of contact for your assigned patient population and care teams. Your day will include:

• Coordinating patient care and follow-up services
• Managing incoming calls, emails, and patient inquiries
• Processing provider orders, including medications, labs, imaging, referrals, and home health services
• Communicating care plan updates with patients, families, community staff, and healthcare partners
• Supporting medication reconciliation and escalating concerns to providers when needed
• Assisting with post-discharge outreach and ensuring appropriate follow-up care is scheduled
• Identifying urgent patient concerns and helping coordinate timely interventions
• Maintaining accurate documentation and patient records
• Supporting telehealth appointments and care coordination activities
• Obtain and document patient vital signs, including blood pressure, pulse, temperature, respiration rate, oxygen saturation, height, and weight, and communicate abnormal findings to the care team as appropriate

What We're Looking For

• High school diploma or GED required
• Medical Assistant preferred but not required
• Strong communication and customer service skills
• Comfortable using computers, email, and electronic medical records

Healthcare experience desired. Experience in healthcare, senior living, medical offices, medical assisting, patient care, or other service-focused environments is preferred. Experience obtaining and documenting patient vital signs is strongly preferred.

Hourly range for posted region: $23 USD - $25 USD
