Brookdale is seeking a Clare Bridge Program Assistant to support person-centered dementia care programs and engage residents in meaningful activities. You will work with the Program Manager to implement a calendar of programs that respect residents’ routines, preferences and life histories.

The role emphasizes collaboration with families and staff to enhance quality of life, with opportunities for growth and advancement within Brookdale’s dementia care community.

Competenze

• High School Diploma or equivalent required.
• Associates Degree preferred.
• Minimum one year of experience with older adults and/or dementia preferred; or equivalent education/experience.

Mansioni

• Assist Clare Bridge Program Manager or Coordinator in leading and implementing person-centered dementia-friendly programs.
• Support residents in seven well-being domains within a structured environment.
• Review resident service plans and complete program documentation per Brookdale standards.
• Ensure dementia care programs comply with state/federal regulations and Brookdale policies.
• Assist with coordinating community events and communicating with families about resident needs.
