Job summary

Brookdale seeks a dedicated Clare Bridge Program Assistant to help execute person-centered dementia-friendly activities within our community. You will support a program calendar, engage residents in meaningful activities, and collaborate with the Program Manager to tailor plans to individual interests and histories.

You will assist in coordinating events, documenting progress, and ensuring quality of life for residents, while promoting Brookdale's Resident and Family Engagement philosophy across

Qualifications

• High School Diploma or equivalent required.
• Associates Degree preferred.
• Minimum one year of experience with older adults and persons with dementia preferred; or equivalent combination of education and experience.
• Must have a valid driver's license and a current 3-year driving abstract with no more than one moving violation.

Responsibilities

• Assist Clare Bridge Program Manager or Coordinator in leading person-centered dementia-friendly programs.
• Support 7 domains of well-being: purposeful, intellectual, social, spiritual, emotional and physical.
• Review resident service plans and complete program documentation per Brookdale standards.
• Ensure programs comply with state and federal regulations and Brookdale policies.
