Job summary

Brookdale is seeking a Clare Bridge Program Assistant to support person-centered dementia programs and resident engagement at our Mesa community. You will help plan and implement activities reflecting residents’ routines, interests, and needs, promoting quality of life in a supportive environment.

As a member of Brookdale, you’ll work with a team to ensure programming aligns with Brookdale’s mission, while developing professionally through ongoing training and internal advancement opportunities.

Qualifications

• High School Diploma or equivalent required; associates degree preferred.
• Minimum one year experience with older adults and persons with dementia preferred.
• Must have a valid driver’s license and a clean driving record (3-year abstract) as condition of employment.

Responsibilities

• Assist in leading person-centered dementia programming across seven well-being domains.
• Review residents’ service plans and document program activities per Brookdale standards.
• Ensure dementia programs comply with state/federal requirements and Brookdale policies.
