Job summary

Sage Creek Post-Acute seeks an experienced nursing leader to oversee resident assessments, MDS completion, and the plan of care. You will coordinate PDPM processes, supervise staff, and ensure compliance with federal and state regulations.

Qualifications include a BSN, current State Board of Nursing licensure, proficiency with Microsoft Office, and PCC knowledge. Prior long-term care supervision is preferred; travel is minimal. Strong communication and data accuracy are essential.

Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain continuing education/licensing.
• 3 years of supervisory experience preferred.
• Long-term care experience preferred.

Responsibilities

• Oversee MDS assessments and ensure timely completion per guidelines.
• Coordinate PDPM processes and weekly meetings for skilled patients.
• Provide MDS training to new coordinators and supervise staff performance.
• Maintain accurate schedules for MDS assessments and data entry for electronic submission.
