Qualifications

• Minimum requirements to perform this position include graduation from an Accredited School of Nursing with a Bachelor of Science Degree in Nursing preferred.
• Must be in good standing with the State Board of Nursing and maintain all required continuing education/licensing requirements.
• 3 years of supervisory experience preferred.
• Long Term Care Experience Preferred.

Responsibilities

• Oversee facility's weekly PDPM meetings for skilled patients.
• Track and monitor facility PDPM tools usage and performance.
• Monitor QM and QASP performance and provide data analysis and recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions.
• Ensure timely completion of all MDS Assessments including PPS Medicare, quarterly, annual, significant change.
• Communicate level of care for new residents to all disciplines.
• Coordinate interdisciplinary participation in completing the MDS for each new admission within regulatory timeframes.
• Schedule and conduct resident care conferences and ensure completion of all MDS reviews.
• Assist disciplines in formulating and revising care plans; set goals and nursing interventions.

Education

Bachelor of Science in Nursing

Licensed State Board of Nursing in good standing
