Job summary

PACS Group is seeking a skilled nursing professional to oversee the facility's MDS and PDPM processes, ensuring assessments are completed per guidelines. You will supervise MDS coordinators, coordinate interdisciplinary care planning, and drive compliance with federal/state regulations.

The ideal candidate has a BSN, at least 3 years of supervisory experience in long-term care, and strong data-analysis skills to optimize outcomes.

Qualifications

• Must possess a BSN from an accredited nursing program and hold/maintain state RN licensure.
• Minimum 3 years of supervisory experience in a long-term care setting.
• Strong knowledge of MDS/PDPM processes and Medicare PPS requirements.
• Proficient with Microsoft Office; PCC experience is preferred.

Responsibilities

• Oversee weekly PDPM meetings for skilled residents.
• Track utilization of PDPM tools and related data.
• Monitor QM and QASP performance with data-driven recommendations.
• Provide PDPM/MDS training to new coordinators.
