Job summary

PACS Group is seeking an experienced nursing professional to oversee facility operations related to resident assessments, MDS, and care plans. The role emphasizes PDPM process oversight, data accuracy, and cross-disciplinary coordination.

Ideal candidates have a BSN, state licensure in good standing, and at least 3 years of supervisory experience in long-term care. The position requires strong data analysis skills and proficiency with Microsoft Office; PCC experience is a plus.

Qualifications

• Bachelor of Science in Nursing required; in good standing with State Board of Nursing.
• 3 years of supervisory experience preferred, long-term care experience preferred.
• Proficiency in Microsoft Office and familiarity with PCC preferred.

Responsibilities

• Oversee facility weekly PDPM meetings for skilled patients.
• Track and monitor utilization of PDPM tools and related metrics.
• Provide PDPM and MDS training to new MDS Coordinators.
• Ensure timely completion of MDS assessments including PPS Medicare, quarterly, annual, and major change reviews.
• Coordinate interdisciplinary participation in MDS and care planning.
• Maintain accurate schedules and data entry for MDS assessments and submissions.
• Ensure confidentiality of resident care information.
