Job summary

PACS seeks a Nursing Supervisor to oversee the facility's resident assessment process, ensure timely MDS activities, and guide the care plan development for residents. The role requires leadership in coordinating multidisciplinary teams and maintaining regulatory compliance.

The ideal candidate holds a BSN, is in good standing with the State Board of Nursing, and has prior long-term care supervisory experience. Proficiency in Microsoft Office and PCC is preferred.

Qualifications

• BSN preferred; licensed in good standing in state board of nursing.
• Experience supervising staff in long-term care.
• Proficiency with Microsoft Office and basic data tools.

Responsibilities

• Oversee facility weekly PDPM meetings for skilled patients.
• Monitor and analyze PDPM tools usage and outcomes.
• Provide PDPM and MDS training to new MDS Coordinators.
• Ensure completion of all MDS assessments (PPS Medicare, quarterly, annual).
• Coordinate interdisciplinary participation for MDS and care planning.
• Maintain confidentiality of resident care information.
