Job summary

MARIONVALLEY in is seeking an experienced MDS/Nurse Supervisor to oversee resident assessments and PDPM compliance. You will lead MDS processes, coordinate data entry, and monitor quality metrics to guide clinical decisions.

The role requires a BSN, an active RN license, and 3 years of supervisory experience in long-term care. Training new MDS Coordinators and ensuring regulatory timelines are essential parts of the job.

Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain continuing education/licensing requirements.
• 3 years supervisory experience preferred; long-term care experience preferred.

Responsibilities

• Oversee the facility to ensure assessments are completed per guidelines, focusing on MDS and resident plan of care.
• Provide oversight on weekly PDPM meetings for skilled patients and track PDPM tool utilization.
• Monitor QM/QASP performance and provide data-driven recommendations.
• Train new MDS Coordinators on PDPM and MDS processes.
• Coordinate MDS data entry, submissions, and regulatory documentation.
• Ensure timely completion of PPS Medicare, quarterly, annual, and significant change assessments.
• Lead care conferences and assist disciplines in formulating care plans.
• Maintain confidentiality of resident care information and ensure regulatory compliance.
