Job summary

PACS is seeking an experienced Nursing Supervisor to oversee the facility’s MDS processes and PDPM compliance in an Ohio post-acute setting. You will supervise MDS coordinators, coordinate interdisciplinary care planning, and ensure timely, accurate documentation per state and federal standards.

Responsibilities include leading weekly PDPM meetings, training new staff, and monitoring performance metrics to drive optimal resident outcomes. A BSN and active state license are required.

Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain continuing education requirements.
• 3 years of supervisory experience, Long Term Care experience preferred.

Responsibilities

• Oversees weekly PDPM meetings for skilled residents.
• Ensures accurate and timely completion of MDS assessments and related processes.
• Coordinates interdisciplinary participation to complete the MDS within required regulatory timeframes.
• Maintains schedules and ensures complete documentation per federal and state standards.
• Generates forms for level of acuity and coordinates submissions as required by state law.
