Job summary

SANANTONIONORTH is seeking a Nursing Supervisor to oversee the resident assessment process, including MDS assessments and plans of care. The role emphasizes PDPM guidance, data analysis, and interdisciplinary coordination to meet regulatory standards.

The ideal candidate has a BSN in good standing with the State Board, at least 3 years of supervisory experience, and Long Term Care familiarity. Proficiency with Microsoft Office and PCC is preferred.

Qualifications

• Bachelor of Science in Nursing (BSN) required; in good standing with State Board of Nursing.
• Minimum 3 years of supervisory experience; long term care experience preferred.
• Must maintain continuing education/licensing requirements.

Responsibilities

• Oversee facility PDPM meetings and resident assessment processes (MDS, plan of care).
• Track and monitor PDPM tools usage and QM/QASP performance.
• Provide PDPM and MDS training to new coordinators.
• Ensure timely completion of MDS assessments and regulatory documentation.
• Coordinate interdisciplinary MDS completion within regulatory timeframes.
• Maintain confidentiality of resident care information.
