Job summary

SANANTONORTH in San Antonio, TX is seeking a Nursing Supervisor to oversee the facility’s MDS and PDPM processes, ensure assessments are completed per guidelines, and coordinate resident care plans.

You will supervise staff, conduct care conferences, monitor QM and QASP performance, and communicate care level changes to all disciplines. Training for new MDS coordinators is a key part of the role.

Qualifications

• BSN preferred and in good standing with State Board of Nursing.
• 3 years of supervisory experience preferred.
• Knowledge of PCC and Microsoft Office.

Responsibilities

• Oversee facility assessments and PDPM/MDS processes.
• Coordinate weekly PDPM meetings and data tracking tools.
• Train new MDS coordinators on PDPM and MDS.
• Ensure timely completion of MDS assessments and care plans.
• Maintain confidentiality of resident information.
• Schedule and conduct resident care conferences and ensure MDS reviews are completed.
• Assist disciplines in formulating and revising care plans to meet resident needs.
• Evaluate care plans for completeness and individuality.
• Notify departments of level of care changes and maintain required records.
