Job summary

PACS is seeking an experienced nursing leader to oversee the resident assessment process, including MDS and PDPM. You will coordinate interdisciplinary teams and ensure timely, compliant documentation for new admissions and ongoing care planning.

The role emphasizes supervising MDS activities, staff training, and maintaining high standards of care within a post-acute setting. Strong data skills and licensure are required.

Qualifications

• Must be a graduate from an accredited School of Nursing with BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain licensure requirements.
• 3 years of supervisory experience preferred; Long Term Care experience preferred.

Responsibilities

• Oversee the facility's weekly PDPM meetings for skilled patients.
• Track and monitor PDPM tools' utilization and data accuracy.
• Monitor QM and QASP performance, providing data analysis and clinical recommendations.
• Provide PDPM and MDS training to new MDS Coordinators.
• Assess health status and level of care for new admissions.
