Job summary

Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain continuing education requirements.

Responsibilities

• Oversee facility weekly PDPM meetings and monitor PDPM tool utilization.
• Ensure timely completion of MDS assessments (PPS Medicare, quarterly, annual, significant change).
• Coordinate interdisciplinary participation in the MDS process and ensure documentation compliance.
