Job summary

MARIONVALLEY seeks a qualified Nursing Supervisor/MDS Coordinator to oversee the facility's resident assessment program, including MDS assessments, care plans, and PDPM processes. You will ensure timely, guideline-conforming assessments and coordinate interdisciplinary care conferences.

You will lead weekly PDPM meetings, monitor QM/QASP metrics, provide training to new MDS coordinators, maintain accurate documentation, and safeguard resident confidentiality.

Qualifications

• Graduation from an Accredited School of Nursing with a BSN preferred.
• Must be in good standing with the State Board of Nursing and maintain required continuing education/licensing.
• 3 years of supervisory experience preferred.
• Long Term Care experience preferred.

Responsibilities

• Oversee the facility's resident assessment program, including MDS assessments, care plans, and PDPM processes.
• Lead weekly PDPM meetings and monitor tool utilization for compliant, effective care.
• Provide data analysis and clinical recommendations to optimize facility outcomes.
• Provide PDPM and MDS training to new MDS coordinators.
• Ensure accurate and timely completion of all MDS assessments and documentation.
