Job summary

San Antonio North Nursing and Rehabilitation seeks an experienced nurse leader to oversee the facility’s resident assessment process, ensuring PDPM, MDS, and plan of care are conducted per guidelines. The role emphasizes data-driven decisions and interdisciplinary collaboration.

The ideal candidate has a BSN, active RN license, and 3+ years of supervisory experience, preferably in long-term care, with strong PDPM/MDS knowledge and regulatory compliance focus.

Qualifications

• BSN preferred; active RN license in good standing.
• Minimum 3 years of supervisory experience; LTC experience preferred.
• Knowledge of PDPM, MDS, and care planning processes.

Responsibilities

• Oversees facility's weekly PDPM meetings for skilled patients.
• Monitors PDPM tools and data accuracy; provides recommendations.
• Trains new MDS Coordinators on PDPM and MDS processes.
• Ensures timely completion of all MDS assessments (PPS, quarterly, annual).
• Coordinates interdisciplinary participation in MDS across departments.
