The role requires a current LVN license, CPR certification, and extensive knowledge of the RAI, OBRA, PPS, and Medicaid reimbursement. You will lead MDS submissions, care plan coordination, and inter-disciplinary communication.

Qualifications

• Current LVN license is required.
• CPR certification required (30 days to obtain if not current).
• Thorough understanding of the RAI process, the Medicare PPS process, and Medicaid reimbursement.

Responsibilities

• Assist the Director of Nursing to ensure documentation meets federal, state and certification guidelines.
• Coordinate the RAI process for timeliness and completeness of the MDS, CAAs, and interdisciplinary care plans.
• Participate in regulatory surveys, report trends to QA, and ensure PPS and MDS timelines are met.
• Initiate and monitor RAI process tracking, discharge/re-entry, and Medicaid tracking forms.
• Ensure MDS information and care delivered is supported by documentation and engage staff as needed.
