Job summary

Apploi in Wichita Falls, TX is seeking an LVN to manage the RAI process from admission through discharge, ensuring clinical compliance and funding from Medicare, Medicaid, and managed care pay sources.

You will complete MDS in a timely, accurate manner, develop individualized care plans, and coordinate with multiple clinical teams to optimize case mix and documentation. Strong teaching and communication skills are essential in this role.

Qualifications

• Must be a Licensed Vocational Nurse (LVN) in good standing and currently licensed by the State.
• Completion of post secondary education or other wise able to read, write and follow oral and written directions at a level necessary to accomplish the job.
• Must be able to relate positively and favorably to residents and families and to work cooperatively with other associates.
• Must be capable of maintaining regular attendance.
• Prefer two years experience in long term care with a good working knowledge of Medicare/Case Management/Medicaid.
• Must have knowledge of RAI Process, PPS requirements, Texas Case Mix, reimbursement forms and the necessary documentation.
• Must have computer skills and proficiency.
• Must have excellent communications and teaching skills in both individual and group settings.
• Must be capable of performing the Essential Job Functions of this job, with or without reasonable accommodation

Responsibilities

• Manage the RAI Process from resident admission to discharge to maintain clinical compliance and funding sources.
• Coordinate assessments and services by other clinical departments; liaise to rehab team and oversee data collection tools.
• Attend daily/weekly meetings to monitor MDS/Care Plans and ensure quality assurance.
• Support facility compliance with State, Federal and company QA standards and resident rights.
