Job summary

Valley View Care Center is seeking a licensed RN/LVN to lead the RAI and MDS processes from admission to discharge, ensuring timely and accurate data in line with policy and federal/state guidelines. You will collaborate with the interdisciplinary team, coordinate MDS assessments, and maintain current MDS status and documentation to support coding and care planning.

Qualifications

• RN or LVN licensed in California.
• 1–2 years nursing experience in a Skilled Nursing Facility preferred.
• Solid knowledge of Case-Mix, Medicare PPS and Medicaid reimbursement.
• Understanding OBRA regulations and MDS terminology.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the Interdisciplinary Team.
• Ensure accurate MDS coding and data entry in the MDS system.
• Monitor compliance with federal and state regulations for RAI processes.
• Maintain current MDS status and audit resident records for accuracy.
• Assist on-floor as needed.
