Job summary

Apploi seeks an MDS Nurse (LPN) to coordinate RAIs and MDS assessments for residents. You will support development of comprehensive care plans, lead care plan meetings, and ensure documentation meets regulatory standards.

You will work with health plans and physicians, schedule ARDs, and audit charts to maintain accurate MDS coding and quality reporting. Strong communication and independent work ethic are essential.

Qualifications

• Active LPN license in good standing.
• RAC training within six months of hire (certification preferred).
• Knowledge of regulations and standards of care for SNF PPS/RAI processes.
• Proficient with Word, Excel, and EMR systems.

Responsibilities

• Coordinate the RAI process under state and federal regulations.
• Complete accurate MDS assessments and supporting documentation.
• Schedule ARDs for payment and OBRA assessments.
• Lead care plan meetings and keep care plans current.
• Transmit assessments and correct errors until accepted.
• Audit charts to confirm documentation supports MDS coding.
• Join triple check meetings before claims are billed.
• Work with health plans and physicians on coverage and approvals.
• Serve as the go-to resource on SNF PPS and quality reporting.
