Job summary

Fundamental is seeking an MDS Nurse (LVN) to coordinate the Resident Assessment Instrument (RAI) process and ensure accurate MDS data for the Interdisciplinary Plan of Care. You will join the nursing management team and participate in PPS ITM meetings to support resident care.

Requirements include a valid LVN license and 1+ year of nursing and MDS/RAP experience, with reliable transportation and ability to navigate state and federal regulations. Competitive benefits apply.

Qualifications

• Must have a current, unencumbered LVN license as required by state.
• 1+ years of direct hands-on nursing and MDS/RAP experience preferred.
• Ability to utilize local and consultant resources in the execution of job responsibilities.
• Ability to develop knowledge of state, federal, and regulatory requirements related to the job.
• Reliable transportation required

Responsibilities

• Coordinating the Resident Assessment Instrument (RAI) process.
• Completing an accurate Minimum Data Set (MDS) and development of the interdisciplinary Plan of Care.
• Participating in the daily Medicare Prospective Payment System (PPS) Interdisciplinary Team Meeting (ITM).
• Establishing and maintaining positive relationships with all members of the healthcare team and functioning as part of the nursing management team.
