Job summary

OPCO Skilled Management seeks an MDS Coordinator in Taos, NM to ensure timely completion of the RAI and care management processes from admission to discharge, following policy and federal/state guidelines.

You will collaborate with the interdisciplinary team to coordinate MDS assessments, ensure compliant documentation, and maintain accurate coding in the facility's MDS systems. RN/LVN licensure and 2+ years SNF nursing experience are required.

Qualifications

• Must be a graduate of an approved RN/LVN program and licensed in the state.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement.
• Thorough understanding of Quality Indicator processes and OBRA regulations.
• Knowledge of the care planning process.
• Experience with MDS 3.0 is preferred.

Responsibilities

• Works in collaboration with the Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews as required by governing agencies.
• Ensures that the Interdisciplinary team decides on completing or not completing additional MDS assessments based on clinical criteria.
• Assist with coordination and management of daily stand up meetings, reviewing resident care and setting assessment reference dates.
• Complies with federal and state regulations regarding completion and coordination of the RAI process.
• Monitors MDS and care plan documentation for all residents; ensures documentation exists to support MDS coding.
• Maintains current MDS status for assigned residents per state and federal guidelines.
• Maintains accurate data entry of resident information into MDS programs.
• Performs accurate coding of the MDS from medical records, observations, and staff input.
• Other duties may be assigned as needed.
