Job summary

OPCO Skilled Management seeks an experienced MDS Coordinator to manage timely and accurate MDS/RAI processes from admission to discharge in alignment with policy, Federal, State, and certification guidelines.

Under the Director of Nursing and VP of Clinical Reimbursement, you may coordinate education and information systems for the clinical department across NM facilities where relocation is required.

Qualifications

• Graduate of an approved Registered Nurse program and licensed.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement.
• Thorough understanding of the Quality Indicator process. Knowledge of the OBRA regulations and Minimum Data Set.
• Knowledge of the care planning process.
• Experience with MDS 3.0, preferred.

Responsibilities

• Collaborate with Interdisciplinary Team to assess resident needs and schedule MDS assessments and care plan reviews.
• Decide on completing or not completing additional MDS assessments based on clinical criteria.
• Coordinate daily stand up meetings to review resident care and assessment reference dates.
• Comply with federal and state regulations for MDS processes.
• Monitor MDS and care plan documentation in the medical record to support coding.
• Maintain current MDS status per state and federal guidelines.
• Ensure timely data entry into MDS programs and finalize MDS data submission to state/federal agencies.
• Assist DON with significant changes, physician orders, and communications reflecting changes in MDS and care plans.
• Prepare scheduling and notices for care planning conferences and communicate outcomes to staff and families.
• Continuously update knowledge on data entry and computer technology.
