Job summary

OPCO Skilled Management is seeking an experienced MDS Coordinator to lead timely MDS/RAI processes from admission to discharge in line with policy, state, and federal guidelines.

Under supervision from Nursing leadership, you'll coordinate interdepartmental education and ensure accurate coding, data entry, and documentation across systems while maintaining compliance and confidentiality.

Qualifications

• RN licensed in the state of practice.
• Minimum of 2 years nursing experience in a Skilled Nursing Facility preferred.
• Strong knowledge of Case-Mix, Medicare PPS and Medicaid reimbursement.
• Understanding of OBRA regulations and the MDS process.

Responsibilities

• Coordinate MDS assessments and care plan reviews per governing rules.
• Monitor MDS and care plan documentation in medical records.
• Maintain current MDS status per guidelines and ensure data entry into systems.
• Communicate with Interdisciplinary Team and schedule care planning conferences.
