Job summary

Outfield Healthcare Partners seeks an experienced MDS Coordinator to ensure timely completion of the RAI and care management process from admission through discharge, in line with policy, federal/state guidelines, and the RAI User's Manual.

Under the direction of the Director of Nursing and VP of Clinical Reimbursement, you will coordinate MDS coding, care plans, and interdepartmental collaboration, maintaining accurate data in the state databases and clinical systems to support reimbursement.

Qualifications

• RN in good standing with state license.
• Knowledge of MDS 3.0, OBRA regulations and PPS.
• Experience with care planning and regulatory compliance.

Responsibilities

• Coordinate MDS assessments and care plan reviews with Interdisciplinary Team.
• Ensure MDS coding accuracy from admission to discharge.
• Maintain MDS status per state and federal guidelines.
• Enter resident data into MDS programs and state databases.
• Attend interdisciplinary and QA meetings and communicate changes.
• Assist with scheduling care planning conferences and staff updates.
