Job summary

Outfield Healthcare Partners is seeking an experienced MDS Coordinator to manage the RAI and care management processes across admission to discharge in New Mexico. You will work under the Director of Nursing and VP of Clinical Reimbursement to coordinate information systems and staff education.

Responsibilities include interdisciplinary MDS scheduling, compliance with OBRA and MDS guidelines, and timely documentation submission to state/federal agencies.

Qualifications

• RN licensed in the state of practice.
• Minimum 2 years nursing experience in a Skilled Nursing Facility.
• Strong knowledge of Case-Mix, Medicare PPS and Medicaid reimbursement.
• Solid understanding of OBRA and MDS processes.
• MDS 3.0 familiarity preferred.

Responsibilities

• Coordinate MDS assessments and care plan reviews with the interdisciplinary team.
• Ensure MDS coding aligns with federal and state regulations.
• Assist with scheduling and coordination of care planning conferences.
• Maintain accurate MDS status and data entry in clinical systems.
• Submit electronic documentation to state databases per policy.
